During the Family Secrets trial, the Robert Grant of the FBI said, "We have 28 made members of the Chicago Outfit roaming in the Chicagoland area. We have over 100 associates of the Chicago Outfit. Incidentally, that's how many have been identified over on the Real Deal forum.

John "No Nose" DiFronzo is generally believed to be the boss. Joseph "The Builder" Andriacchi is believed to be underboss/street boss. While the Outfit once had 7 crews, in recent years it had been said to be down to 4, and most recently down to 3 crews according to Ross Rice of the FBI. DiFronzo's brother, Peter, and Rudy Fratto are believed to head the Elmwood Park/North Side crew. The Carusos are believed to head the 26th Street/South Side crew. Until recently, there was also the Grand Avenue/West Side crew and the Cicero/Melrose Park Crew. If the Outfit is down to 3 crews now, there has been some more downsizing, and it may be a case of North side, West side, and South side. Even though he was listed as being part of the North Side in the past, most recently John "Pudgy" Matassa has been said to oversee the Cicero/Melrose Park crew....which might be combined under the West Side now.

The Outfit continues to be involved in sports betting, video poker machines, loansharking, burglary rings, etc. Very little involvement in the drug trade though. And not much labor racketeering union activity remaining. Also some legitimate businesses like DiFronzo's D&P Construction, Andriacchi's interests in the Rosebud restaurant chain, strip clubs, etc.
